CCR is currently seeking a professional and self-motivated Early Intervention Service Coordinator to serve Butler County families.
Provide service coordination to children and families who are eligible for Early Intervention services under Pennsylvania Act 212 and the Pennsylvania Office of Child Development and Early Learning regulations and procedures.
Essential Functions:
Serve as the single point of contact in helping parents of children birth to age 3 with developmental delays in obtaining Early Intervention Services Locate, coordinate and monitor services for Early Intervention families in their natural settings, such as family homes, childcare, and community locations throughout Butler County Maintain accurate records, documentation and utilization requirements Screen and track children who are at-risk for a developmental delay Communicate effectively with direct supervisor and co-workers.
Be able to work as a team and facilitate meetings with parents and interdisciplinary team members to ensure supports and services are in line with assessed needs and family goals Offer families opportunities and support for the infant or toddler with a disability to participate in community activities with other children, inform family of appropriate community resources, and assist in arranging for the infant or toddler with disability to receive medical or health services as needed This position is Monday - Friday, daylight.

Qualifications:
Bachelor's degree with successful completion of 12 college credits in sociology, social welfare, child development, early childhood education or special education, elementary education or other related social sciences.
One year working with families or individuals with disabilities.
ACT 33/34 and FBI clearances, valid driver's license and proof of car insurance coverage must be provided prior to hire.
